Program SummaryÌý

Over the next five years, the Elevance Health Foundation will focus grantmaking support on efforts to improve maternal and infant health outcomes. Nonprofits with a history of successful community initiatives are invited to apply.Ìý

The Foundation will prioritize funding for programs that improve maternal/infant health, target disparities, address social needs, and remove barriers to care throughout the pregnancy journey—from pre-conception support to postnatal care for mothers and babies.

Funding is available for initiatives that support one or more of the following goals:

  • Reducing disparities in pre-term birth rates
  • Reducing disparities in severe maternal morbidity (SMM) incidence
  • Improving maternal/infant health outcomes

For CU Boulder, projects must be designed as national or multi-state initiatives. Colorado-only projects are not eligible under this funding opportunity.Ìý

Internal Application Requirements (all in PDF format)

  • Project Summary (2 pages maximum): Identify goals and anticipated outcomes aligned with the following areas: 1) Improving pre-term birth rates among socially vulnerable populations; 2)ÌýReducing SMM incidence (adverse outcomes) among socially vulnerable populations; and 3) Improving maternal/infant health outcomes across the continuum of care, including access to and utilization of pre-conception, prenatal/delivery, and postnatal services.

  • Budget Overview (1 page maximum): A basic budget outlining project costs is sufficient; detailed OCG budgets are not required.Ìý

Eligibility

Elevance Health Foundation will consider proposals from qualified nonprofit organizations across the U.S. with an emphasis on:Ìý

  • National programs promoting scalable and sustainable systemic change, orÌý
  • Local programs. Implementing relevant interventions in California, Florida, Georgia, Indiana, Missouri, Nevada, New York, Ohio, Texas, and Virginia.
  • For CU Boulder, projects must be designed as national or multi-state initiatives. Colorado-only projects are not eligible under this funding opportunity.Ìý

Limited Submission Guidelines

CU Boulder may submit only one application as the Lead Organization.Ìý

Award Information and DurationÌý

The amount requested should be primarily limited to costs directly related to the implementation of the proposed program, and may include support for measurement, evaluation, and reporting. The grant term will be 1-3 years based on need.ÌýÌý

  • Support whole-person health
  • Address the health-related social needs of those being served
  • Include interventions that are culturally relevant to those being served
  • Build trust through programming that considers the unique needs of those being served
  • Offer innovative, scalable solutions
